Forms

Students need to complete all the necessary forms under the guidance of their Adult Sponsor in order to compete in the Intel-affiliated Contra Costa Science & Engineering Fair.

PRE-APPROVAL : All students and Adult Sponsors should refer to the Scientific Review Committee checklist to determine whether your project needs pre-approval before beginning experimentation (checklist here). Projects requiring pre-approval will also need additional forms submitted. This should be apparent after completing Form 1.

Another approach would be to use the Intel ISEF Rules Wizard. This "wizard" asks a series of questions about your planned project and when you're done, it will provide a list of forms that you need to complete. If you want the official rules, the following index links to a full-text version of the 2008 Intel International Science and Engineering Fair (ISEF) Rules and Regulations.

The purpose of pre-approval is to simply make sure that the experiment is completely safe and in compliance with the ISEF Rules and Regulations so it will be important that students be descriptive in their Research Plans and all other necessary forms so the application doesn't get returned.

All projects that require pre-approval may be submitted at any time, but no later than Friday, February 1st. Remember that experimentation may not begin until the SRC has approved your project.

Forms below are in .pdf format. If you are unable to download and/or print the forms on this website, please let us know your form needs via e-mail or by phone at 925.682.7474 Ext. 3133. You can obtain a free download of Adobe Acrobat Reader to access the .pdf documents below.
